Trillium Senior Consultants was built on a simple belief: senior living organizations don't need someone to take over — they need a partner who's done the work firsthand and knows how to help it bloom.

Working alongside operators, we help organizations achieve better outcomes for the seniors they serve — not through outside theory, but through decades of hands-on experience across the full spectrum of senior living operations, from caregiving on the floor to executive oversight of multi-state portfolios.

Our Story

Meet Sarah Silva

A woman with long wavy hair, wearing a navy blazer and white blouse, sitting on a beige armchair and smiling at the camera in a professional setting.
A group of seven diverse people standing together and smiling in front of a sign and an American flag. They are dressed in professional attire, standing inside a building with cream-colored walls and wood accents.

Sarah Silva brings more than 20 years of senior living experience to organizations navigating operational challenges, regulatory change, growth, leadership transitions, and the pursuit of stronger resident outcomes.

Her perspective is grounded in firsthand experience at every level of senior living. Sarah began her career as a caregiver and med tech before advancing through community and corporate leadership roles, including Memory Care Administrator, Executive Director, and Division President. In her most recent executive operating role, she oversaw a 26-community portfolio across six states, encompassing Independent Living, Assisted Living, Memory Care, and multi-level campus environments.

Throughout her career, Sarah has helped organizations translate ambitious goals into practical operating systems. Her experience includes leading operational evaluations and improvement initiatives, developing and stabilizing new communities, supporting more than 20 acquisitions, strengthening regulatory and quality performance, building leadership teams, and creating scalable memory care programs. She directed the development and stabilization of a 300-unit luxury senior living campus and has helped communities achieve deficiency-free surveys and national quality recognition, including the AHCA/NCAL Silver National Quality Award.

Sarah is nationally recognized for her leadership in assisted living quality and public policy. She serves as Board Chair of the National Center for Assisted Living, representing providers nationwide and contributing to federal and state policy discussions. She has also chaired the NCAL Quality Committee, served as an outside examiner for the AHCA/NCAL National Quality Award program, and serves on the board of the Alzheimer’s Association. Her contributions have been recognized with the NCAL Jan Thayer Pioneer Award and the Oregon Health Care Association Special Service Award.

As President of Trillium Senior Consultants, Sarah works alongside owners, operators, executives, and community leaders to understand what is happening beneath the surface, establish clear priorities, and build solutions that can be successfully implemented and sustained. Her approach is candid, collaborative, and grounded in the realities of senior living operations. She brings an objective perspective without losing sight of the people affected by every decision—residents, families, and the teams who serve them.”
